Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 10/29/25, Staff #1 threw a remote across the classroom at Child #1 (2-years-old) which struck the child on the forehead. Staff reported the child cried and had a visible red mark on her forehead. The child was given an ice pack to treat the injury.
Plan of Correction: Per the Center: ?Due to a medical emergency a staff employee experienced beyond her control, a remote was thrown across the room hitting a child in the forehead showing red for two hours leaving no visible marks with an icepack applied. When management was made aware of the incident the employee had left to go by ambulance to the emergency room. The center notified parents immediately, called licensing to self report and reported to CPS. Tiffany's Learning Center does not condone this type of behavior even if a medical emergency occurs. The employee will not return to TLC. Our children's protection, health and safety is our #1 priority. TLC has had all employees fill new documentation forms to disclose any medical conditions or health problems that may interfere with fulfilling their job responsibilities at TLC and a new medical questionnaire form has been implemented for all employees.?

The center is required to ensure the center's own policies and procedures that are required
by these standards are in compliance.

The center's discipline procedures state "Tiffany's Learning Center does not condone or tolerate the use of physical punishment of any kind on TLC property."

On 10/29/25, Staff #1 threw a remote across the classroom at Child #1 (2-years-old) after Child #1 pulled the daily calendar off of the wall. The remote hit Child #1's forehead.
Plan of Correction: Per the Center: ?At the time of hiring the employee, they did not disclose any medical information, due to being medically cleared for 5 years. The question was asked during the hiring process and put in the computer about medical conditions, and had been misplaced in the computer. To make sure this doesn't happen in the future we have implemented additional onboarding documentations and health questions to be signed and a physical copy put in their files. TLC will encourage potential employees to disclose any medical condition or health problems that may interfere with fulfilling their job responsibilities at TLC and a new medical questionnaire form has been implemented for all current employees.?

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Documentation, interviews, and video footage reviewed showed that on 10/29/25, Child #1 (2-years-old) pulled the daily calendar off of the classroom wall. Staff #1 asked her to stop and threw a remote across the classroom at her. The remote hit the child's forehead.

Staff #1's interactions with Child #1 were not constructive in nature and were not intended to
redirect the child to appropriate behavior.
Plan of Correction: Per the Center: "On 20-29-25, a staff employee was redirecting child #1 and another member listed as staff #1 entered the classroom and redirected the child #1. While doing this they had a medical emergency. All staff at TLC have been properly trained on redirecting a child. Tiffany's Learning Center does not condone this type of behavior even if a medical emergency occurs. The employee will not return to TLC. Our children's protection, health and safety is our #1 priority.?
Standard 8VAC20-780-70
A record shall be kept for each staff person that contains the required information.

The record of Staff #1, who had been employed for three and a half months, did not have any information about any health problems that may interfere with fulfilling the job responsibilities.
Plan of Correction: Per the Center: "We have placed a hook to attach and detach the calendar when not in use after circle time daily. TLC does not condone or tolerate the use of any physical punishment of any kind. Even if an employee has any type of medical condition. TLC has implemented additional onboarding documentations and health questions to be signed and a physical copy put in a employees files. TLC will encourage potential employees to disclose any medical condition or health problems that may interfere with fulfilling their job responsibilities at TLC and a new medical questionnaire form has been implemented for all current employees.?
