Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 01/29/2025, as part of the licensure period.
The on-site inspection began at 3:00 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above.
There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site.
This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 02/10/2025. A POC submitted after this date will not appear on the public website.
Violations
10There were no immunization records for Child #2, Child #3, Child #4 and Child #5, who have been in care for seven weeks.
There were no physical examination in the records for Child #2, Child #3, Child #4 and Child #5, who have been in care for seven weeks.
Staff #1 and Staff #2 had not completed the required training.
On the day of inspection, no staff had completed training to perform a daily health observation of children.
The Supplemental page of the inspection report dated 11/19/2024, which contains confidential information, was posted on the premises of the program.
The first aid kit did not contain the following: scissors, tweezers, triangular bandages, gauze pads, thermometer and a first aid instructions manual.
There was no documentation that evacuation drills were practiced in the months of December 2024 and January 2025.
The records for Child #1, Child #2, Child #3 and Child #4 enrolled for seven weeks, did not contain the following information: Name, home address, and home phone number of each parent who has custody. Child #2, Child #3 and Child #4 did not contain the addresses of two designated people to call in an emergency. There was no record available for Child #5.
There were five children present but the attendance record had only four children registered as present.
There was no signed acknowledgment in the records for Child #1, Child #2, Child #3, Child #4, and Child #5 of having received the required written agreements.
