An unannounced, on-site monitoring inspection was initiated and completed on 01/29/2025, as part of the licensure period.
The on-site inspection began at 3:00 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above.
There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The agreement shall be signed by the parent and include and include an authorization for emergency medical care, a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible, and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
