Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/07/2024, as part of the conditional licensure period. The on-site inspection began at 9:30 am and ended at 12:25 pm. The inspector reviewed 5 children?s records and 5 staff records. There were 13 children present and 6 staff.

The inspection included: document review; tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 2, who has been attending for 3 weeks, did not have documentation of an immunization record on file.
Plan of Correction: Going forward, will get immunizations prior to the 1st day of attendance.
Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff 1 was alone with a group of three infants and was not certified in CPR or first aid.
Plan of Correction: Will be attending a class this weekend.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment and maintained in the staff record.

Staff 4 and Staff 5, who have been working at the center for over a month, did not have any completed reference verifications on file.
