Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 07/21/2025. The on-site inspection began at 10:40 am and ended at 1:35 pm. The inspector reviewed 5 children?s records and 5 staff records. There were 38 children present and 8 staff.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

The storage closet in Classroom 5, which contained various cleaning materials, was not locked.
Plan of Correction: Will keep the door locked.
Standard 8VAC20-780-430-B
Materials and equipment available shall be age and stage appropriate for the children.

There were small pieces of a wooden block set in Classroom 1 (age 6 months-16 months) and Classroom 2 (age 16 months-24 months). The manufacturers recommended age for the set of blocks, as indicated on the container, was 24 months and up. Children in both classrooms were observed placing pieces of the block set into their mouths.
Plan of Correction: Will ensure that all toys in classes are age and stage appropriate. Will discuss with staff.
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and water and sanitized after each use; and disposable diapers shall be disposed of in a system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Staff 1 did not sanitize the diapering surface after changing a diaper. Staff 1 used their hand to open the diaper disposal system rather than using the foot-operated method.
Plan of Correction: Will review policy and procedures.
