Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 03/10/2025 and completed on 03/10/2025, as a part of the conditional licensure period. The on-site inspection began at 4:20pm and ended at 5:15pm. The inspector reviewed compliance in the areas listed above. There were no children present and one caregiver. The inspector reviewed one caregiver's and one household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
