During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 4/21/25 and completed on 4/24/25, as a part of the conditional licensure period. The on-site inspection began at 1:40pm and ended at 2:50pm (from 12:15-1:30pm on 4/24/25). The inspector reviewed compliance in the areas listed above. There were four children present and one caregivers; the point total was 10. The inspector reviewed three children?s records and one caregiver's record, and two household members' records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/6/25. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a
conspicuous place on the licensed premises.
The results of the most recent inspection were not posted, which is required.
Plan of Correction: Moving forward, the provider will post inspections.
Standard 8VAC20-800-100-A
Repeat Violation
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
The records for Child #2 and Child #3 did not contain documentation of a physical examination. Both children have been enrolled for more than 30 days.
Plan of Correction: Provider has obtained the physicals for the children.
Standard 8VAC20-800-120-B
Assistants' and substitute providers' records shall contain all the required information.
On the day of the inspection, a household member was left alone supervising and assisting with the children in care. The record for the household member, who was used as an assistant, did not contain all the required information for an assistant.
Plan of Correction: Provider will complete a record for the household member to qualify as an assistant, on a need basis.
Standard 8VAC20-800-150-D-1
An assistant 18 years of age or older who is left alone with children in care shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate
to the age of the children in care.
A household member, who was used as an assistant on the day of the inspection, did not have current certification in CPR.
Plan of Correction: Provider has hired a qualified assistant that will be working alongside the provider. CPR/First aid training will be scheduled for the household member and put into their record.

Standard 8VAC20-800-240-A
Repeat Violation
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe and operable condition.
The door of a safety gate at the home was broken and the mesh side of a playpen was ripped. Both, the safety gate and the playpen, were not in a safe or operable condition.
Plan of Correction: Provider has replaced the gate and the playpen has been removed.
Standard 8VAC20-800-320-A
Small electrical appliances such as, but not limited to, curling irons, toasters,
blenders, can openers, and irons shall be unplugged.
On the day of the inspection, a toaster in the kitchen was plugged in.
Plan of Correction: Moving forward, the toaster will be unplugged completely.

Repeat Violation
A child?s record shall contain all the required information.
The records for Child #1, Child #2, and Child #3 were missing information that is required to be in children?s records. There was no record for review for Child #4.
