Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 5/14/2025. The on-site inspection began at 11:15am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 15children present with 3 staff. The inspector reviewed 4 staff records on-site on 5/14/2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program

All employees shall undergo a background check in accordance with subsection B prior to employment and every five years thereafter.

Employee #1 did not have documentation of a subsequent Central Registry background check (search of the child abuse and neglect registry) on file. Staff #1's last Central Registry results expired over 1 year ago.
Plan of Correction: We will work to obtain the results of the central registry within 10 business days or as soon as possible.
