During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/23/25, as a part of the conditional licensure period. The on-site inspection began at 10:15am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. As of the date of the inspection there were no children enrolled. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/7/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A gallon of bleach and a shopping bag containing additional cleaning products were on the floor in the kitchen. The kitchen door was not locked.
Plan of Correction: All hazardous materials will be locked up and placed in a safe place.
Chain locks were installed on the outer and inner sides of the kitchen door.
Standard 8VAC20-780-280-G
If hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents. There was an unlabeled spray bottle with a liquid substance on the sink in the bathroom.
Plan of Correction: All containers will be properly labeled and stored.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. An evacuation drill was not practiced in March.
Plan of Correction: A fire drill will be implemented once a month and recorded properly.
