During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/8/25.The on-site inspection began at 9:50am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 4 children present and 2 staff. The inspector reviewed 4 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/24/25. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #1, began working before the results of the criminal history check were received.
Plan of Correction: Facility responded: Ensure that the physical copy of the National criminal background report is in the employee folder prior to start date.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who had been enrolled for more than 30 days..
Plan of Correction: Facility responded: Children will have a physical on file before starting or within 30 days of enrollment. Parent of child #4 stated she set appointment (7/18/2025) Parent was informed child can only return once he receives physical.
Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #1 and child #3 was not signed and the complete dates of immunizations received were not visible.
Plan of Correction: The facility responded: Informed Parents of Child #1 & child #3 that clear and visible immunization record needs to be provided immediately.
Standard 8VAC20-780-270-A
Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. A tall bookcase in the foyer was unsecured and leaning away from the wall poising a topple hazard.
Plan of Correction: The facility responded: The bookcase was anchored to the wall.
Standard 8VAC20-780-420-E-1
For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child. The record shall contain the amount of time the infant slept, the amount of food consumed and the time, and a description of bowel movements. There was no daily record containing required sleep and feeding information for child #4.
Plan of Correction: Facility responded: Updated daily sheets have been placed in infant room for staff and parents.
Standard 8VAC20-780-530-A
There shall be at least two staff members with current certification in cardiopulmonary resuscitation (CPR) and first aid present on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care. There was only one staff member in the facility with CPR and first aid certification.
Plan of Correction: Facility responded: Another employee who is certified came in to replace caregiver. Employee was not allowed to return to work until CPR training was completed.
