During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/19/26. The on-site inspection began at 11:00am and ended at 1:05pm The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 staff. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/3/26. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child 3 (17 months old) last documented immunization was over seven months ago.
Plan of Correction: The facility responded: All children under two years old records will be reviewed every six months. Make sure all immunization records are dated and signed. Informed parent of child three to bring updated shot record.

Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. The protective belt was missing from a high chair. A diaper changing pad was torn. There was standing water on the playground.
Plan of Correction: The facility responded: High chair was removed until belts can be replaced or fixed. Diaper changing pad was replaced. Before children are allowed in the playground area all water is removed.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children. There were unlocked cleaning solutions, antibiotic ointment, and interior paint in multiple areas of the facility.
Plan of Correction: The facility responded: All hazardous materials will be store in a safe and secure space at all times. Reiterate policy with staff concerning hazardous materials.

When centers choose to provide meals or snacks, a menu listing foods to be served during the current one week period shall be dated and list any substituted foods. The menu posted was not dated and substituted foods served were not listed.
Plan of Correction: The facility responded: Weekly menus will be posted and dated. If substitutions are made, they will be written in on the menu.
Standard 8VAC20-780-560-G
When food is brought from home the food container must be clearly labeled and dated in a way that identifies the owner. There was a bottle that did not contain a date or the name of the child that had used it.
Plan of Correction: The facility responded: All sippy cups, bottles and food brought from home will be properly labeled with name and date upon arrival to center.
Standard 8VAC20-780-570-A
When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Child 1 and Child 2 were in high chairs without the protective belts fastened.
Plan of Correction: The facility responded: Informed staff to be sure to fasten children into highchairs properly using safety belts when placing them in highchairs.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2 was missing required information.
Plan of Correction: The facility responded: Make sure all records for children contains all required information including start date, physical including immunization records.
