Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 02/14/2025 as part of the conditional licensure period. The on-site inspection began at 9:00 am and ended on 10:40 am. The inspector reviewed compliance in the areas listed above. There were 3 children (1 resident and 2 program children) present and 2 caregivers; the point total was 8. The inspector reviewed 4 children?s records, and 4 caregiver/household members records on-site.

The inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination within 30 days after the first day of attendance.

The record for Child 3 and Child 4, both of which have been attending for more than 30 days, did not contain documentation of a physical examination.
Plan of Correction: Will get the information from the parents.
Standard 8VAC20-800-120-B
Assistant and substitute provider?s records shall contain the information required by the standards.

The record for Caregiver 2 and Caregiver 3 did not contain all required information.
Plan of Correction: Will make assistant/substitute caregiver records today.
Standard 8VAC20-800-200-A
The provider shall orient the substitute provider and assistant by the end of their first week of assuming job responsibilities.

Caregiver 1 verified that Caregiver 2 has not received orientation training. Caregiver 2 was observed in the role of Substitute Caregiver on 02/14/2025.
Plan of Correction: Will do the training today.

REPEAT VIOLATION

Hot water at taps available to children shall be maintained within a range of 105-120 degrees Fahrenheit.

The water at the kitchen sink measured 122 degrees Fahrenheit.
Plan of Correction: Will turn the temperature down more today.

Children?s records shall contain the information required by the standards.

Each of the four children?s records were missing various components.
Plan of Correction: Will get the necessary paperwork from the parents.
