Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 05/09/2025. The on-site inspection began at 10:00 am and ended on 11:25 am. The inspector reviewed compliance in the areas listed above. There were 4 children (1 resident and 3 program children) present and 2 caregivers; the point total was 12. The inspector reviewed 5 children?s records, and 2 caregiver/household members records on-site.

The inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION

The provider shall obtain documentation of a physical examination within 30 days after the first day of attendance.

The record for Child 1, who has been attending for more than 7 months, did not contain documentation of a physical examination.
Plan of Correction: Will ask the parent to get a copy.
Standard 8VAC20-800-540-A
When infant carriers are occupied by a child, a safety strap shall be used and securely fastened.

Child 1 was in an infant carrier without the safety strap securely fastened.
Plan of Correction: Will make sure that the straps are fastened.

REPEAT VIOLATION

Children?s records shall contain the information required by the standards.

The records for Child 1, 2, 3 and 5 were missing required information.
Plan of Correction: Contacting the parents today and will get the information.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.

Medication 1 was on the desk in the living room.
Plan of Correction: The medication was locked during the inspection.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

Emergency evacuation procedures were not practiced in March 2025 nor April 2025.

Bottles shall be labeled with the child?s full name and the date, if more than one infant is in care.

There were two unlabeled and undated bottles on the desk in the living room; there were 2 infants in care during the inspection.
