Inspector Notes

An announced, on-site initial inspection was initiated and completed on 10/22/2024 as a part of the conditional licensure period. The on-site inspection began at 2:00pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was (7). The inspector reviewed 3 children?s records, 1 caregiver and 1 household member's records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Applicants/providers must obtain a completed national criminal background check for household members at 18 years old.

Household Member #1 (over the age of 18) did not have results of fingerprinting in the record.
Plan of Correction: Provider had documentation that the results are being processed, as of 10/11/2024.

Initial applicant/provider must obtain results of a search of the central registry for the applicant and household members over the age of 14.

Furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.

The fire extinguisher was inoperable, on empty.
Plan of Correction: Provider ordered a new fire extinguisher.

First Aid kit emergency supplies shall be in the family day home that contain required supplies, including triangular bandages and a First Aid instruction manual.

The family day home's first aid kit did not contain more than one triangular bandage and a First Aid instruction manual.

Meals and snacks provided by family day home shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food
Program of the United States Department of Agriculture (USDA). A fruit and a vegetable or two different vegetables should be on the lunch menu.
Lunch provided by family day home, did not have a fruit or second vegetable listed on menu.
Plan of Correction: Provider adjusted menu to include a second vegetable or a fruit at lunch.
