During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site monitoring inspection was initiated and completed on 7/3/2025, as a part of the licensure process. The on-site inspection began at 10:00am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 14. The inspector reviewed 4 children?s records and 1 caregiver record on-site.
This inspection included: document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver had 8.5 hours of training documented in file during the past year.
Plan of Correction: Provider will complete remaining/required 6.5 training hours by the end of July. Provider will keep track of training hours, to ensure 16 hours are obtained annually.
Standard 8VAC20-800-240-A
Areas of the family day home, shall be maintained in a clean, safe, and operable condition.

The home only had smoke detector on the main level.
Plan of Correction: Provider will install smoke detectors on each level and in bedrooms by 7/7/2025.
Standard 8VAC20-800-320-G
Wood burning fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.

The provider did not have documentation of an inspection and stated they were unaware of need for inspection. as they do not plan to use the fireplace.
Plan of Correction: Provider will have chimney inspected by a knowledgeable inspector and permanently closed.
Standard 8VAC20-800-330-A
A landline telephone, that does not require electricity to operate, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

The home did not have a landline or landline telephone. at the time of inspection, during hours of operation, with children present.
Plan of Correction: Provider has telephone company scheduled for 7/10/25 land line installation. Provider has a landline telephone to use.
Standard 8VAC20-800-430-A
Children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.

The protective barrier to the stairs did not prevent access of a child under two years of age as observed during inspection.
Plan of Correction: Provider to install gate that prevents children's access to stairs by 7/7/2025.
Standard 8VAC20-800-430-D
Protective barriers or guardrails on sides of stairs shall be constructed to prevent a child from climbing over, crawling or falling through, or becoming entrapped.

Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

The family day home did not obtain immunizations for child #3 or child #4, who both turn 2 while in care, every six months.
Plan of Correction: Provider requesting documentation of immunizations from parents at pick up today. Provider will track ages/when immunizations are coming due and request from parents.
