Repeat Violation:
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

Child #3, who had been enrolled for 12 months, did not have a physical examination on record at the time of inspection.

In classroom 1, a part of the wall had chipping paint and cracked and broken drywall exposing metal edges.

In classroom 1, two cans of cleaning materials were observed on the counter unlocked and accessible to children.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child 1 has a diagnosed food allergy and there was no allergy care plan from the physician, which was overdue by three months.
