An announced, on-site initial inspection was conducted 10/1/2024 from 12:00 - 1:45pm. The inspector reviewed compliance in the areas listed above. There were no children present, 1 provider, and 1 household member. The inspector reviewed 1 providers record and 1 household
members records on-site. This inspection included document review, interviews, observations, and a tour of the home.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
