During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 1/31/2025 as a part of the conditional licensure period. The on-site inspection began at 8:25 am and ended at 9:45 am. The inspector reviewed compliance in the areas listed above. There were five children present and four staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The license and the last inspection documents were not posted.
Plan of Correction: The Director will ensure all required postings are posted in a conspicuous location on the licensed premise.
Standard 8VAC20-780-40-G
The center shall meet the proof of child identity and age requirements as stated in 22.1-289.049 of the Code of Virginia.

Child #2, who has been enrolled since December, 2024, did not have proof of identity in their record.
Plan of Correction: The Director will ensure that proof of child identity is acquired and placed in the record. If proof of identity is not obtained, the Director will ensure the proper notification is completed.
