Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 05/09/2025 and completed on 05/09/2025 as a part of the conditional licensure period. The on-site inspection began at 9:50 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 staff. The inspector reviewed 3 children's records and 2 staff records on-site.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child 1 lacked documentation of immunizations. Child 1 was in care and has been enrolled for more than 30 days.
Plan of Correction: The director stated that current immunization documentation has been requested and will be requested again.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
The immunization documentation for child 2 does not include documentation of updated immunizations once every six months. The most recent immunizations on file for child 2 were administered more than 10 months ago.
Plan of Correction: The director stated that current immunization documentation has been requested and will be requested again.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
Child 1 lacks documentation of a physical exam. Child 1 was in care and enrolled more than 30 days ago.
Plan of Correction: The director stated that a physical has been requested and will be requested again.
Standard 8VAC20-780-270-A
Outside areas and equipment of the center shall be maintained in a safe condition:
The outside hand railing has areas of peeling paint. The basketball hoop moves when pushed and three accessible weeds were observed on the outdoor play area.
Plan of Correction: The director stated that hand railing will be sanded and or painted. Weeds will be removed and water added to base of basketball hoop.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
A bottle of hand sanitizer was observed on top of the toilet in the bathroom.
Plan of Correction: Hand sanitizer was locked up during the inspection.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities are maintained in compliance with the terms of the current license issued by the department.
Two children under the age of 30 months were cared for in a classroom without an exit door to the outside. The terms of the current license requires children under age 30 months to be cared for in an area where there is an exit door to the outside.
Plan of Correction: The director stated that space will be reorganized to ensure children under age 2 1/2 years are in care in the front classroom.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required elements.
The enrollment record for child 1, 2 and 3 lacked complete emergency contact information.
Plan of Correction: The director stated that each parent will be asked to provide complete emergency contact information.
