Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
The facility did not maintain a current written list of children's sensitivities. Child 1, who was in care, has a food sensitivity listed on the enrollment form.
Plan of Correction: Food sensitivity will be noted throughout the center.
Standard 8VAC20-780-430-I
If combs, toothbrushes, or other personal articles are used, they shall be individually assigned.
None of the children's water bottles were labeled with each child's name.
Plan of Correction: Bottles should be labeled daily.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required elements.
1. The enrollment record for child 1 lacked the home address and phone number for one parent and a street address for both emergency contacts.
2. The enrollment record for child 2 lacked a work phone number for one parent, name of physician and phone number and allergy information.
3.The enrollment record for child 3 lacked a work phone number for each parent and place of employment for one parent.
4. The enrollment record for child 4 lacked documentation of annual review and update by the parent.
Plan of Correction: Will review record keeping and increase file reviews.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include all required elements.
The enrollment record for child 1 lacked the written parent agreement. Child 1 was in care during the inspection.
