Vendors shall maintain, and keep at the family day home, information for each enrolled child that includes the names of persons other than the custodial parent who are authorized to pick up the child. This information was not contained in the records of Child #1 or Child #3.

Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course. This training shall be related to child safety, child development, health and safety in the family day home environment, and any required department sponsored training. Caregiver #1 completed 7.5 hours of training in the last year and did not attend the department's health and safety update course.

Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. There was no foot operated trash can in the bathroom. Dirty diapers were observed in the bathroom in an open trash can with no lid.

Toilet chairs, when used, shall be emptied promptly, cleaned, and sanitized after each use. The toilet chair in the bathroom was observed to be dirty with dried feces and dried toilet paper in the bowl.

(Repeat Violation) The family day home must have one working battery-operated radio. There was no working battery-operated radio in the home.

The provider shall practice shelter-in-place procedures twice a year. Caregiver #1 did not practice 2 shelter-in-place drills in the last year.

The provider shall practice lockdown procedures at least annually. Caregiver #1 did not practice a lockdown drill in the last year.

(Repeat Violation) The provider failed to ensure that the criteria on the health and safety checklist was met at the time of the inspection. Section 1. Emergency evacuation drills must be completed monthly. There was no fire drill completed in 12/2025. Section 5. Proof of each child?s identity is required. There was no documentation that proof of identity was reviewed for Child #2.

(Repeat Violation) The provider shall maintain on file a signed statement from each parent, affirming receipt of the information to parents statement. There were no signed information to parents statements in the records of Child #1, Child #2, or Child #3.

(Repeat Violation) The provider shall maintain an individual record for each child enrolled in care that includes name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care. This information was not contained in the records of Child #1, Child #2, or Child #3.

(Repeat Violation) The provider shall maintain an individual record for each child enrolled in care that includes the results of the health examination obtained within 30 days of enrollment. The physical examination for Child #3 was overdue by more than 8 weeks.
