During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/18/2025. The on-site inspection began at 2L00 PM and ended at 2:45 PM.. The inspector reviewed compliance in the areas listed above. There were 7 children present and 1 caregiver; the point total was 15. The inspector reviewed 7 children?s records and 2 caregiver/household member records on site.
This inspection included :

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance has been 65 days since the first day of attendance of child 6. Results of a physical examination under the direction of a physician have not been obtained,
Plan of Correction: The provider responded with the following:
Immunization records will be obtained for all children prior to the children being allowed to attend.
Standard 8VAC20-800-60-B
A child's record shall contain the name, address and telephone numbers of two designated persons to contact in an emergency if a parent cannot be reached.
The record for child 6 did not include the street address for the two emergency contact persons.
Plan of Correction: The provider responded with the following:
All children's records will include complete street addresses for emergency contact persons.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
The immunization record in the file of child n6 did not have the child's name and was not signed by a physician or his designee.
