Inspector Notes

An in-person initial subsidy inspection was conducted on 9/24/2024 from 9:44am to 10:55am. There was no children in care with the vendor. The inspector reviewed (3) adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Provider's must obtain a completed Sworn Statement from each household member prior to residing in the home, a National Fingerprint Check Result for each household member by the end of the 30th day of residence for new household members and the Central Registry results must be obtained by the end of the 30th day of residence for new household members.

Household Member #3 did not have the Sworn Statement, Fingerprint Check results or the Central registry results during the inspection and has been residing in the home for 35 days.

inspection, this was confirmed by the inspector on the same day.

The children's play kitchen was missing the sink and faucet and was exposing 2 sharp screws.
