During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 9/08/2025. The inspection started at 12:45pm and concluded at 1:55pm. There was one child present and one caregivers; the point total was four. The inspector reviewed two children?s records and four caregiver/household members records on-site on 9/08/2025.

This inspection included document review, tour of the home, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-180-B
The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Child #1 was present and had arrived at approximately 7:00am. The vendor had not taken attendance today and had not documented the child's arrival as it occurred.
Plan of Correction: I will make sure to take attendance everyday as it occurs.
Standard 8VAC20-790-190-B
The child's information shall include the following required information: Proof of the child's identity.

Child #1 and Child #2 did not have proof of the child's identity.
Plan of Correction: I will ask the parents for proof of birth today.
Standard 8VAC20-790-260-C-2
Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children in areas used by children of preschool age or younger.

One unused outlet beside the crib was not covered with a protective cover..
Plan of Correction: I will put covers on all unused outlets.
Standard 8VAC20-790-360-M
Toys or objects hung over an infant in a crib and crib gyms that are strung across the crib may not be used for infants older than five months of age or infants who are able to push up on their hands and knees.

A mobile toy was hanging over an infant in a crib that was over 7 months old.
Plan of Correction: I removed the mobile immediately.

The diapering surface shall be nonabsorbent and washable and cleaned and sanitized after each use.

The diapering surface the vendor was using had a blanket on the changing table with a roll of paper and was not nonabsorbent and washable.
Plan of Correction: I will buy a diaper changing pad.
