Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/14/2025. The on-site inspection began at 9:00 am and ended on 10:00 am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver; the point total was 17. The inspector reviewed 5 children?s records, and 3 caregiver/household members records on-site.

The inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-570-A
The provider shall ensure that a caregiver does not exceed 16 points.

Caregiver 1 had a total of 17 points in care on 10/14/2025.
Plan of Correction: Will dismiss a child to reduce the point total.
Standard 8VAC20-800-60-B
Children?s records shall contain all of the elements as required by the standards.

The records for Child 1 and Child 2 did not contain all of the elements required by the standards.
Plan of Correction: Will get the information today.
Standard 8VAC20-800-90-A
Documentation of immunizations shall be obtained prior to the child?s first day of attendance.

The immunization record for Child 1 was obtained more than 2 weeks after the child?s first day of attendance.
Plan of Correction: Will make sure to get before the 1st day.

Documentation of obtaining additional immunizations is required once every 6 months for children under the age of two years.

The immunization record for Child 5 was not updated every 6 months until the child turned 2 years old; the updated immunization record was overdue by 5 months.

Bottles shall be refrigerated and labeled with the child?s full name and the date, if more than one infant is in care.
