During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused monitoring inspection was initiated on 6/12/25 and completed on 6/12/25, as a part of the conditional licensure period. The on-site inspection began at 1:05pm and ended at 1:35pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two staff. This inspection included tour of the facility and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION
Where playground equipment is provided, resilient surfacing must comply with minimum safety standards and must be under and surrounding the equipment with climbing apparatus to create a fall zone free of hazardous obstacles.

The resilient surfacing under and around the playground equipment did not meet the minimum safety standards. The mulch measured less than two inches in depth and did not adequately cover the required fall zones beneath and surrounding climbing equipment. The mats used did not meet the minimum safety qualifications for resilient surfacing and were not large enough to cover the designated fall zones.
Plan of Correction: We decided to remove the playground set.
Standard 8VAC20-780-450-A
Cribs, cots, mats and beds used by children must have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Child A was laying on a cot with no top cover or bottom cover.
Plan of Correction: Will ask parents to bring top and bottom cover for Child A.
