Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/11/2026 and completed on 3/11/2026, as a part of the licensure period. The on-site inspection began at 9:48am and ended at 11:29am. The inspector reviewed compliance in the areas listed above. There were twelve children present and three staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review and a tour of the facility.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must maintain a written record of children's serious and minor injuries in which the record must include any future action to prevent recurrence of the injury.

The center's minor injury record for child C did not include any future action to prevent recurrence of the injury.
Plan of Correction: Will make sure to include any future actions to prevent recurrence of injuries on the written injury record.
