Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on October 6, 2025. The inspection started at 10:00am and concluded at 11:00am. There were two children present and one caregiver; the point total was six. The inspector reviewed four children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
1The record for Child #1 was missing the name, home address, and telephone number for each parent who has custody, name, address, and telephone number for each custodial parent's place of employment, name address, and telephone number of at least one designated person by the parent to contact in case of an emergency, and names of persons other than the custodial parent who is authorized to pick up the child. Child #1 has been in care for three months.
