During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 12/19/2024 and completed on 01/07/2025 in response to a self-report received by the licensing office on 12/05/2024 relating to forbidden action. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/20/2025. A POC submitted after this date will not appear on the public website.
Violations
3On 12/03/2024, staff #1 was supervising a room of preschool children. Staff #1 carried child #1 (age three years) into the classroom sideways by the waist and placed the child down on the floor to speak with the child when staff #1 grabbed the child?s cheeks. Child #1 cried due to being grabbed by the cheeks. Another staff in the room consoled the child after the incident. It was reported that child #1 did not want to return to daycare that next day after the incident.
In addition to new employee orientation, a monthly training plan has been developed to engage in ongoing professional development. Below is the agenda topics per month:

The center?s discipline policy in the parent handbook states: ?Magnolia Ridge permits NO PHYSICAL PUNISHMENT OF ANY KIND or withholding of food, drink, or bathroom privileges. Our teachers practice cause and effect discipline methods and under no circumstance yell, threaten, or belittle children in any manner.?
The center?s discipline policy in the staff handbook states: ?Inappropriate forms of discipline are to never verbally or physically abuse a child. Always encourage positive self-esteem. Communicate with children on their level. Never yell at children.?
