During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted on 10/14/25. The inspection started at 10:13am and ended at 11:15am. A language translator assisted with communications during the inspection. There were 2 children present and 1 caregiver, the point total was 8. The inspector reviewed 3 children?s records and 1 caregiver record while on-site. The inspection included document review, tour of the day care areas of the home and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
4The record for Child #1 did not include verification of the child's proof of identity. The child has been enrolled since March 2025.
The record for Child #3 did not include documentation that the parent had received and signed the sick child policy. The form in the child's record was blank and had no signature.
There was no documentation (training certificate) that the provider has completed the 3 hour training in the previous 12 months.
A soiled diaper was observed lying on a shelf above the diaper changing table.
There was no documentation of emergency drills practiced in the last 12 months.
