During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 02/24/2025, as a part of the conditional licensure period. The on-site inspection began at 11:00 a.m. and ended at 1:15 p.m. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
10There was no sworn statement for Staff 2, who had been employed for over two months.
Staff 2, who had been employed for over two months, did not have a central registry search requested. Staff 2 was working alone with children.
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child 1, who had been attending for over one week, did not have documentation of an immunization record on file.
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.
There was no screening for Staff 1, who had been employed for over one month and there was no screening for Staff 3, who had been employed for over two months. The screening for Staff 2 was completed was completed 13 days after hire.
There was no documentation of orientation training for Staff 1, employed for over one month, Staff 2, employed for over two months, and Staff 3, employed for over two months.
There was no staff member on duty who has obtained instruction in performing the daily health observation.
Staff 1 was alone with a group of 4 children, ages 16 month- 2 years, and was not certified in CPR or first aid
There was only one staff member currently certified in CPR and first aid on premises during the inspection.
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The records for enrolled Child 1, Child 3, and Child 4, were each missing several of the required components.
Staff records shall be kept for each staff person that contain all of the required information.
The records for Staff 1, Staff 2, and Staff 3 were each missing several of the required components.
