SYSTEMIC VIOLATION
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The immunization for Child 2 (enrolled three months) was obtained one day after the child first attended the center and Child 5 (enrolled one month) did not have documentation of an immunization on file.
Plan of Correction: Provider will obtain information from parents.

SYSTEMIC VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There were no physical examinations in the records of Child 2 (enrolled for three months), Child 3 (enrolled for seven months) or Child 5 (enrolled for over 30 days).

For centers offering multiple shifts, a qualified program director or qualified back-up director shall regularly be on site at 50% of the day shift and at least two hours during the evening shift.

During the evening shift, a qualified program director is not on site for the required two hours.

The administration of medication shall be performed by a staff member who has satisfactorily completed medication administration training.

There is no documentation that Staff 1, who administered medication 1 to Child 4 on multiple occasions, has completed medication administration training.

During the center?s hour of operation, one adult on the premises shall be in charge of the administration of the center. This person shall be either the administrator or an adult appointed by the licensee or designated by the administrator.

Upon arrival, staff were unable to identify the administrator on-site. The identified administrator arrived approximately five minutes later.
Plan of Correction: An administrator will aways be assigned and onsite.

Centers shall be in compliance with the center?s own policies and procedures required by these standards.

The center?s medication policy states that medication will not be administered. Staff 3 administered medication to Child 4 on each day of their attendance during the months of June, July and August 2025.

There shall be at least two staff members who have current certification in CPR and first aid present on the premises during when children are in care.

Upon arrival, Staff 1 and Staff 2 were the only staff on premises. Staff 1 and 2 are not certified in CPR or first aid. Staff 3, who is certified in CPR and first aid arrived five minutes later. The center was previously cited on 6/03/2025 for not having staff certified.
Plan of Correction: Staff will receive training.

The first aid kit did not include adhesive tape or the first aid instructional manual.
