SYSTEMIC VIOLATION
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff 1, who had been employed for two months, did not have a completed criminal background check.
The criminal background check for Staff 4, who had been employed for four months, was dated nine days after their first day of employment.
Plan of Correction: The provider has requested background checks.

SYSTEMIC VIOLATION
The center must request a search of the central registry prior to hire and a provisional staff shall be supervised at all times by a person who received a qualifying result on a background check.

Staff 1, who had been employed for two months, did not have a central registry search requested.
The central registry search for Staff 2, employed for four days, was requested three days after hire. Staff 2 was alone with children.
Plan of Correction: Provider will obtain at hire.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

There was no documentation of a negative TB screening for Staff 1, employed for two months.
Staff 4 had a TB screening dated 9 days after employment.
Plan of Correction: Provider will ensure staff are doing all required screenings.

Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

There was no documentation of orientation training for Staff 1, employed for two months.

Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing all of the required information.

There was no documentation of required information provided in writing to Staff 1, employed for two months.
Plan of Correction: Provider will make sure orientation is done.

REPEAT VIOLATION
There shall be at least two staff members who have current certification in CPR and current certification in first aid on the premises during the center?s hours of operation, on field trips, and wherever children are in care.

Upon arrival, Staff 1, Staff 2, and Staff 3 were the only staff for less than 2 hours at the center and were not certified in CPR or first aid. The center was previously cited on 12/02/2025 for not having staff certified.
Plan of Correction: Staff will get CPR training

The first aid kit did not include singular use gloves or the first aid instructional manual. The center was previously cited on 12/02/2025 for missing required items.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The records for Child 1, enrolled seven months, Child 3, enrolled 1 year, and Child 4, enrolled 11 months, were each missing several of the required components. The records for Child 1 and Child 4 were previously cited on 12/02/2025 for missing required components.
Plan of Correction: Provider will make sure that all children files are updated. If not will not allow children to return.
