Upon arrival, 6 children (ages 15 months- 4 years) were asleep in Room 2 out of actual sight and sound supervision of staff. Staff were in and out of Room 2, often at the same time, leaving the children out of actual sight and sound supervision on multiple occasions.
Staff 4 left two preschool age children sitting on a bench in the hallway outside the restroom out of actual sight and sound supervision of staff for approximately one minute. The rear exit of the facility is visible and accessible from the bench in the hallway.
