When staff are supervising children, they shall always ensure their care, protection and guidance.

On multiple occasions, during the months of November and December 2025, an independent contractor, acting in the role of a staff member and required to meet standards for staff, was alone with and transported school age children enrolled in the center. Required background checks that protect children were not completed before staff were alone with children. Required staff orientation, including training on emergency procedures for transporting children, was not completed. Health information on the staff and qualifications as a driver was not documented.

The center did not ensure proper care and protection of four children who were transported in a personal uninsured vehicle on 03/02/2026. Two children (ages 5 years and 6 years) required child restraints that were not in the vehicle. Additionally, the transportation emergency procedure requirements for transporting children and required documentation were not in the vehicle.

When children are in ongoing mixed age groups, the staff to child ratio and group size applicable to the youngest child in the group shall apply to the entire group.

Two staff were supervising 12 children (ages 22 months-9 years) in Room 1. On multiple occasions during the inspection, for durations up to approximately one minute, Staff 2 left the room leaving Staff 1 supervising all 12 of the children; a total of 3 staff were required.

Before the child?s first day of attending, parents shall be provided in writing with the centers transportation policy.

On 03/02/2026, enrolled children were transported to the center and the center did not have a transportation policy.

When centers choose to provide meals or snacks, the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

The center served individual packaged chips as the afternoon snack, which do not qualify as a creditable component of a snack. Two components are required to be served as a snack.

SYSTEMIC VIOLATION
Any vehicle used by the center for the transportation of children shall meet the following requirements: vehicle shall be insured with at least the minimum limits established by Virginia state statues.

There was no insurance documentation for Vehicle 1, used to transport children on 03/02/2026.

SYSTEMIC VIOLATION
The center shall ensure that during transportation of children the Virginia state statutes about safety belts and child restraints are followed as required by the Code of Virginia and that the following information is in the transportation vehicles: emergency numbers; and the center?s name, address and phone number.

On 03/02/2026, Child 3 (age 6 years) and Child 4 (age 5 years) were transported in Vehicle 1 without a child restraint device.

The required emergency information was not in Vehicle 1 used to transport children.

Parental permission for transportation and field trips shall be secured before scheduled activity.

On 03/02/2026, Child 1 (age 9 years), Child 2, (age 8 years), Child 3 (age 6 years) and Child 4 (age 5 years) were transported in Vehicle 1 without documentation of parent permission.

On multiple occasions, during the months of November and December 2025, children were transported by an independent contractor without documentation of parent permission.

During the inspection, there were three enrolled children present at the center whose arrival was not documented on the attendance record.
