Inspector Notes
An unannounced, on-site initial inspection was initiated on 12/16/2024 and completed on 12/16/2024. The on-site inspection began at 10:12 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were no enrolled children at the time of the inspection. The inspector reviewed 1 caregiver and 4 household members' records electronically on 12/16/2024. This inspection included document review, a tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/21/2024. A POC submitted after this date will not appear on the public website.
Violations
3There was a hole in the ground in the outdoor play area, which posed a tripping hazard.
A brick patio was observed in the middle of the outdoor play area that had several unleveled bricks, posing a tripping hazard.
There were large tiles at the bottom of the outdoor play area stairs that were not leveled, posing a trip hazard.
All the unleveled bricks on the patio area were already fix it.
The two large tiles in the outdoor area by the stairs were already leveled.
The family day home did not have triangular bandages.
The family day home does not have documentation of a written emergency preparedness and response plan.
