During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 4/3/2025 and completed on 4/4/25 as a part of the conditional licensure period. The on-site inspection began at 3:00pm and ended at 5:15pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 staff. The inspector reviewed 5 children?s records and 3 staff records on-site on 4/3/25 and electronically on 4/4/25. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/21/25. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The records for child #1, child#2, child #4, child #5 did not contain documentation of immunizations.
Plan of Correction: Staff responded: Each child entering care will have an immunization record on day 1 going forward.
Standard 8VAC20-780-340-C
During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency. On 4/3/25 there was only one staff member on duty at the facility. The facility did not have an immediately available staff member, volunteer or other employee.
Plan of Correction: The facility responded: Site will ensure that a staff member is readily available at all times.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's own policies and procedures are followed. On 4/3/25 the facility?s staffing plan requiring two staff members on site and communication with a custodian as a point of contact in case of emergency was not followed.
A. Staff #1 was the only staff member onsite. Staff #1 was supervising three children alone, for approximately 90 minutes.
B. Staff #1 did not notify the custodian on duty that this person was needed as a point of contact in case of emergency.
Plan of Correction: Facility responded: If alone, staff member will coordinate with a custodian or another school personnel to provide coverage.
Standard 8VAC20-780-530-C
There shall be at least two staff members present who have current cardiopulmonary resuscitation (CPR) and current first aide certification during the center's hours of operation, and whenever children are in care. On 4/3/25 there was only one staff member present with current CPR and first aid certification during the first 90 minutes of child care.
Plan of Correction: Facility responded: At least 2 staff will be on site with CPR/1st aide certifications.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for child #4 and child #5, were missing the following required components: name and phone number of the children's physician, the address of 2 emergency contacts and the communicable disease written agreement required in 8VAC20-780-90-A3.
Plan of Correction: Each child enrolled will have completed medical emergency contacts and communicable disease information.
