Inspector Notes

An announced, on-site initial inspection was initiated and completed on 12/09/2024. The on-site inspection began at 9:50am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were zero children present and two staff. The inspector reviewed zero children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-70
Staff records shall be kept for each staff person with all of the required information. The records for Staff #1, Staff #2, Staff #3, Staff #4, and Staff #5 did not contain documentation of a name, address, and telephone number of a person to be notified in an emergency and information about any health problems that may interfere with fulfilling the job responsibilities, which shall be kept at the center.
Plan of Correction: We will add the information to each staff records and collect the required information for all staff members going forward.
