Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/6/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:50 and ended at 12:15. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 13. The inspector reviewed 3 children?s records and 1 caregiver records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.

The provider does not have the attendance records for the children in care for any other date than the date the inspection occurred.
Plan of Correction: I tossed the papers that I had used for attendance so will make sure I keep them.
Standard 8VAC20-800-60-B
Information regarding parents, emergency contacts and insurance information must be contained in the record.

Child 1, Child 2 , Child 3 and Child 4 are missing parents work information or address,
Child 1, Child 2 and Child 3 are missing the addresses for the emergency contacts.
Child 2 and Child 3 are missing medical insurance information.
Plan of Correction: I will ask the parents for the information.
Standard 8VAC20-800-70-A
The provider must provide parents with the information regarding liability insurance and the information for parents form.

Child, 1,2,3 and 4 do not have the information for parents form or the liability insurance form in the files.
Plan of Correction: I gave the information for parents to the parents but will make sure I get a copy and keep it in the files.
Standard 8VAC20-800-900-A
To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise.

The provider has children in care that are two and younger and served them uncut grapes for lunch. youngest child is 16 months.
Plan of Correction: I cut the grapes up after you told me and will make sure I do that for any food that is on the list.
