During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/3/2025, as a part of the conditional licensure period. The on-site inspection began at 10am and ended at 1pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 7 staff. The inspector reviewed 5 children?s records, and 1 new staff record on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/11/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center inside shall be maintained in safe condition.
1 - There is cabinet in the three-year-old room, approximately 3 ? feet tall that is not secured to the wall and pose a toppling hazard.
2 - There is a dangling cord in the school age room, within reach of children and pose an entanglement hazard.
3 ? An infant was observed sleeping in a crib with items including a pacifier on a cloth string which poses an entanglement hazard. A boppy pillow and bib hanging over the side of the crip, a second bib on the mattress of the crib, and a toy in the infant?s hand.
4 ? There is a pack-n-play in the two-year-old room that is used every morning for infants until the infant room teacher comes to bring them to the infant care room.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. A five-year-old child was sent to the restroom alone. Staff #2 stood in the door way of the classroom. The child walked through four doorways and outside sight and sound supervision of Staff #2.

Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water before and after a diaper change. Staff #1 did not wash her hands before or after changing the diaper of child in care in the 2 year-old room.

Standard 8VAC20-780-500-B
When diapering there shall be sight and sound supervision for all children when a child is being diapered. The diaper changing table in the infant room faces a wall. There were four infants in care with one staff member. The staff could not maintain sight supervision of the other three infants present while changing the diaper of one child.

Standard 8VAC20-780-520-C
If diaper ointment or cream is used, the following requirements shall be met, written parent authorization noting any known adverse reactions and a record shall be kept that includes the child's name, date of use,
frequency of application and any adverse reactions. Staff #3 stated that she had administered diaper cream to Child #1 on more than one occasion. However, there was not any documentation of written parent authorization or documentation as to when the diaper cream was applied.

Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. Raisins were served to a child in care in the infant room. Children present for care ranged in age from 7 to 22 months old. Raisins were accessible to all four children present in the room.
