During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 3/27/2025, as a part of the Conditional licensure period. The on-site inspection began at 9:30 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were (6) children, including provider's own child, present and (1) caregiver; the point total was (15). The inspector reviewed (6) children?s records and (1) caregiver record on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children's records are required to contain name, address and telephone number of two designated persons to contact in an emergency.
T he record for Child #1, 2, 3, and 4 was missing a second name, address and telephone number for emergency contact. The children were present during the inspection and had been in care for 3 months

Plan of Correction: Parents will provide a second emergency contact for each child.
Standard 8VAC20-800-70-B
Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the policies for the administration of medications.
There were no parent acknowledgements available for six children who have been in care for 2 and 3 months.
Plan of Correction: All parents will sign the second page acknowledging they have received the policy on medication administration.
