During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 3/19/2025, as a part of conditional licensure period. The on-site inspection began at 10:10 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 13 children present and 5 caregivers. The inspector reviewed 2 children?s records and 6 caregiver records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent.

The record for Child A did not have a documentation of parent receiving written agreement regarding authorization of emergency medical treatment, ill child policy, or reportable communicable diseases. Child A has been in attendance 1.5 months.
Plan of Correction: We will have dad sign at pick up this afternoon.
