During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 3/19/2025, as a part of conditional licensure period. The on-site inspection began at 10:10 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 13 children present and 5 caregivers. The inspector reviewed 2 children?s records and 6 caregiver records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The record for Child A did not have a documentation of parent receiving written agreement regarding authorization of emergency medical treatment, ill child policy, or reportable communicable diseases. Child A has been in attendance 1.5 months.
