Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/01/2025 and completed on 10/01/2025. The on-site inspection began at 12:05PM and ended at 12:40PM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver. The inspector reviewed 4 children?s records and 1 caregiver/household member record. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

The 3 hour health and safety update course was last taken 9/13/2024. The new training should've been completed by 9/13/2025.
Plan of Correction: The training will be completed by the end of the week.
