Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 02/21/2025 as part of the conditional licensure period. The on-site inspection began at 11:58 am and ended on 1:35 pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 caregivers; the point total was 28. The inspector reviewed 5 children?s records, and 4 caregiver/household members records on-site.

The inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Assistant and substitute caregiver records shall contain the components required by the standards.

The records for Caregiver 2, Caregiver 3, and Caregiver 4 were missing required components.
Plan of Correction: Will get the information completed.
Standard 8VAC20-800-200-A
The provider shall orient the substitute provider and assistant by the end of their first week of assuming job responsibilities.

Caregiver 2, Caregiver 3 and Caregiver 4, all of which have been working in the family day home since 11/2024, have not received orientation.
Plan of Correction: Will complete orientation training with them on Monday.
Standard 8VAC20-800-250-A
Hanging items including window blind or curtain cords shall be out of reach of children under five years of age.

There was a blind cord within the reach of Child 4 (10 months old) while the child was observed lying in their crib.
Plan of Correction: The cord was moved during the inspection and will be taped to ensure it stays out of reach.

A child?s record shall contain the components required by the standards.

The records for Child 4 and Child 5 were missing required components.
Plan of Correction: Will get the information from the parent.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child?s full name and the date, when more than one infant is in care.

There were 5 infants in care on 02/21/2025; a total of 3 unrefrigerated and undated bottles were observed during the inspection.
Plan of Correction: Will be sure to date and keep in the refrigerator.
