Inspector Notes

An unannounced inspection was initiated on 05/13/2025 and completed on 05/22/2025 in response to a complaint received by the licensing office on 05/12/2025 relating to allegations of the care of children. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-140-D
A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.

Caregiver 1 confirmed that there was no documentation of Caregiver 2 serving as the substitute provider on 05/12/2025.
Plan of Correction: I have gotten a substitute provider sheet out and it will be completed each time.
Standard 8VAC20-800-540-A
When infant carriers are occupied by a child, a safety strap shall be used and securely fastened.

On 05/22/2025, Child 1 (age 7 months), was in an infant carrier without the safety strap securely fastened.
Plan of Correction: Will make sure to always strap children in.
Standard 8VAC20-800-570-A
The provider shall ensure that a caregiver does not exceed 16 points.

On 05/12/2025, Caregiver 2 was the only caregiver providing supervision to 11 children (ages 6 months-3 years), totaling 33 points, for approximately 25 minutes. A total of 3 caregivers were required for this number.
Plan of Correction: Will make sure to have enough staff.

Caregivers shall wash their hands with liquid soap and running water before feeding or helping children with feeding.

On 05/22/2025, Caregiver 2 did not wash their hands before serving Child 2 (age 12 months) a bottle.
Plan of Correction: Will always wash hands before and after serving bottles.
