Inspector Notes
An unannounced inspection was initiated on 09/15/2025 and completed on 09/23/2025 with a local agency in response to a complaint received by the licensing office on 09/11/2025 relating to allegations of the care of children. The inspector reviewed 1 caregiver record and 1 household member record on-site. The inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration, physical health of caregivers and household members, care of children, and the Code of Virginia and are documented on the violation notice issued to the program.
Violations
5The sworn statement for Caregiver 1 was dated 4 days after their first day of employment.
Caregiver 1, employed for more than 3 months, did not have documentation requesting a child abuse and neglect check from one state.
The record for Caregiver 1 did not contain documentation that character references were checked before employment.
Household Member 1 did not have a Tuberculosis screening.
The provider shall ensure that a caregiver does not exceed 16 points.
09/11/2025, Caregiver 4 was the only caregiver providing direct care and supervision to a total of 19 points for approximately 15 minutes until Caregiver 3 arrived. Caregiver 2 confirmed that they were in the home but not providing direct care and supervision to the children that day.
