During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on February 18, 2025, and completed on February 18, 2025, as a part of the conditional licensure period. The on-site inspection began at 2:05 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was seven. The inspector reviewed two children?s records and one caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/21/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-480-B
Equipment and materials used by a child shall be appropriate to the age, size, ability, and interest of the child. Child #2, four years in age, was observed in a highchair for snack. The provider stated she has her in the highchair because she makes a mess. The licensing inspector stated child #2 was to old and big to be sitting in the highchair. Child #2 responded ?yeah, I?m a big girl, I go to school.?

Standard 8VAC20-800-540-A
When occupied, highchairs are required to be used according to the manufacturer?s instructions and a safety strap shall be used and securely fastened. Child #2 was observed in a highchair without the safety strap securely fastened.

Standard 8VAC20-800-60-B
The provider shall ensure a child?s record contains all required documentation. The record of child #1 and child #2, both enrolled for 21 days, did not contain the name, address and telephone number of each custodial parent?s place of employment; the policy number of the child?s medical insurance; and the address of one designated person listed to be contacted in case of an emergency.
