When a child's clothing or diaper becomes wet or soiled, the child shall be leaned and changed immediately. Child #1 did not have his pull-up checked after he used the bathroom. The last documented check was 11:45 a.m. The child was picked up at 5:30 p.m. with and the pull up was wet.
