Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/8/2025, as a part of the conditional licensure period. The on-site inspection began at 2:45pm and ended at 4:05pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 31. The inspector reviewed 5 children?s records and 4 caregiver/household members records onsite on 10/8/2025.This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-A
The provider is required to maintain an up-to-date record at the family day home for each enrolled child.

Child #1, #2, and #5 were missing documentation of Authorization for emergency medical care. Child #3 and Child #4 were missing the name, phone number and address for 1 of 2 required emergency contacts.
Plan of Correction: We will get the missing information and place in the child records.
Standard 8VAC20-800-960-E
Bottles are required to be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

There were two infant bottles for two different infants that did not have the date on them during the inspection.
Plan of Correction: We will make sure all bottles have the date moving forward.
