Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 04/22/2025 and completed on 04/22/2025. The on-site inspection began at 10:20 am and ended at 12:15 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver; the point total was 16. The inspector reviewed 6 children's records on-site.

Standard 8VAC20-800-240-A
Repeat Violation

Outside areas of the family day home shall be maintained in a safe condition.
Cracked and broken plastic was observed on two pieces of outdoor play equipment.
Plan of Correction: The plastic table was repaired. The push toy was discarded.
Standard 8VAC20-800-60-B
Repeat Violation

Children's records lacked all required elements.
The enrollment record for child 1, 3, 4 and 5 lacked complete emergency contact information.
The enrollment record for child 1 and 4 lacked the work phone number for one parent.
Plan of Correction: The provider stated that each parent will be asked to provide complete information for enrollment records.
Standard 8VAC20-800-890
When food is brought from home, the food container shall be clearly labeled in a way that identifies the owner.

A cup of milk with attached nipple, for child 1, was not labeled to identify the owner.
Plan of Correction: The provider stated that all food/cups of milk will be labeled with child's name upon arrival each morning.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Child 2 lacked documentation of required immunizations.
Plan of Correction: The provider stated that the parent will be asked to provide the current immunization record.
Standard 8VAC20-800-900-B
Children shall not be allowed to eat or drink while walking, running and playing.

Child 1 was observed walking around the child care room while drinking milk from a cup with attached nipple.
Plan of Correction: The provider indicated that children will be held or sitting down at a table / chair when drinking from cups.
