Inspector Notes

An announced, on-site initial inspection was initiated and completed on 12/9/2024, as a part of the licensure process. The on-site inspection began at 10:00am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was11. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/24. A POC submitted after this date will not appear on the public website.

The provider must request a search of the central registry prior to the employee's first day of employment.

Caregiver #1 did not have central registry search results. Provider was working alone with children.

The family day home shall keep a written record of children in attendance each day.

The family day home did not have a written record of children in attendance available at inspection.

Standard 8VAC20-800-60-B
The provider shall maintain an up-to-date record at the family day home for each enrolled child that contains name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Emergency contact addresses were not in the records For Children #1, 2, 3 and 4.

Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of Operating information to include all required information.
